Marriage, remarriage, death and the effect on 401(k) distributions

It’s common enough: An employee marries and names their spouse and children as the beneficiaries of their 401(k) account. The employee’s spouse dies and the employee remarries but fails to update their beneficiary designations. Then the employee dies. To whom do you distribute their 401(k) assets?
